چکیده

The treatment and outcome of cervical dystonia (CD) with myelopathy (CDM) varies among studies. We retrospectively reviewed the clinical features, neuroimages, methods of treatment, and modified Rankin scale (mRS) score of patients with CDM in our hospital. There were seven male and three female patients, with a mean age of 53 years. The mean age at onset of CD was 24.9 years, with a mean interval from dystonia to the diagnosis with myelopathy of 28.1 years. The dominant level of cord compression was at C3/C4 in seven patients, C4/C5 in one patient, and C5/C6 in two patients. Four of five patients with an initial mRS ≥ 3 remained moderately to severely disabled after surgery, with a follow-up periods of 3-5 years, and one of them experienced recurrent cage dislocation and neurological deterioration due to an unstable spine. Perioperative stabilization is important to minimize neurological sequelae. Whether surgical intervention improves the functional outcome of patients with moderate-to-severe disability demands further randomized controlled studies.

Idiopathic dystonia and cervical spondylotic myelopathy.

Cervical myelopathy developed in two patients with idiopathic torsion dystonia. There were marked spondylotic changes in both patients, probably attributable to the incessant dystonic movements of the neck. Previous cervical spine surgery may have exacerbated the myelopathy in one of the patients. Cervical myelopathy complicating idiopathic dystonia must be distinguished from other causes of ne...

Bilateral C5 Palsy Following a Circumferential Surgery for Cervical Spondylotic Myelopathy: Case Report and Review (In press)

Background and Importance: The development of C5 root palsy is a well-known potential complication of cervical spine surgeries for the correction of cervical spondylotic myelopathy. It typically occurs unilaterally but on extremely rare occasions it might occur as a bilateral anomaly. The value of intraoperative neurophysiologic monitoring in detection of iatrogenic acute versus delayed onset C...

Dystonia and Peripheral Nerve Surgery in the Cervical Area

Cervical dystonia is the most common form of focal dystonia (Dashtipour et al., 2007). It is characterized by involuntary movement of the neck resulting in abnormal neck posture (Brin & Benabou, 1999; Dent, 2002). Cervicalgia and headache sometimes occur in patients suffering from the disease (Albanese, 2005; Brashear, 2004, Schim, 2006). A critical long-term sequelae of this kind of movement d...

Reoperation Rates After Anterior Cervical Discectomy and Fusion for Cervical Spondylotic Radiculopathy and Myelopathy: A National Population-based Study.

STUDY DESIGN National population-based cohort study. OBJECTIVE To compare the reoperation rates between cervical spondylotic radiculopathy and myelopathy in a national population of patients. SUMMARY OF BACKGROUND DATA There is an inherently low incidence of reoperation after surgery for cervical degenerative disease. Therefore, it is difficult to sufficiently power studies to detect differ...
